Director, Population Health Services Organization

The Director, Population Health Services Organization (PHSO) is responsible for overseeing, managing and development of the team of Associate Directors, Liaisons, and Specialists that comprise the PHSO. This responsibility includes oversight & development of appropriate strategies and action plans for Tandigm PCPs and their staff, focusing on driving their clinical, operating, and financial performance consistent with Tandigm’s goals and objectives. Additionally, this role will work collaboratively with Tandigm’s Medical Directors and functional leadership and teammates across Care Delivery, Clinical Networks, Quality, Finance,Informatics, and Accurate Coding and Education to ensure that the network is sufficiently supported.

Essential Functions

  • Consistently exhibits behavior and communication skills that demonstrate Tandigm’s commitment to superior customer service, including quality, care and concern with each and every internal and external customer.
  • Ensure an understanding of broader Tandigm strategic initiatives and they impact and implications of these strategies on the next generation operating model.
  • Manage, develop, and support the team of Associate Directors, Liaisons, and Specialists that comprise the PHSO.
  • Coach and mentor external facing teammates on practice interactions by observing practice visits, reviewing practice presentations and communications and providing ongoing feedback.
  • Participates in the design and implementation of Tandigm initiatives, including program, product and technology projects, around improving physician service response/problem resolution, improving operational efficiency, reducing costs and improving provider satisfaction.
  • Ensure network needs and feedback are heard and appropriately escalated. Communicates areas of accountability and performance expectations to the internal functional team. Ensures that key goals and objectives are accomplished in keeping with established priorities and timeframes; sets priorities and re-calibrate priorities on an ongoing basis dependent on the needs of the business.
  • Support the development and documentation of practice success plans to improve Tandigm’s clinical, operating, and financial performance.
  • Participate in the prioritization, communication planning, and facilitation approach pertaining to practice visits conducted by Medical Directors and Clinical Network Liaisons.
  • Engage Tandigm’s primary care network to collect feedback on the quality and effectiveness of the practice touchpoints and continuously explore ways to improve the experience.
  • Supports the Vice President of PHSO and the Chief Medical Officer with other tasks or requests as needed.
  • Remain engaged in learning about the best practices of Population Health and value based care. Seek out best practices, lessons learned, and best means to engage resources.
  • Keep abreast of emerging technologies as it relates to enabling new business capabilities.
  • Uses, protect, and disclose Tandigm’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards and Tandigm’s enterprise Integrity policies.
  • Ability and availability to travel to sites within the five county Philadelphia region (Bucks, Montgomery, Chester, Delaware & Delaware), on a flexible schedule, as required.
  • Performs additional duties as assigned.

Education and Experience


  • Bachelor’s and/or Master’s Degree in Business Administration, Communications, or other related field or equivalent work experience with;


  • Minimum 6-8 years of related work experience, with at least (minimum) 3 years of experience in leading cross-functional teams.
  • Demonstrated understanding of the healthcare industry, specifically Providers, Health Plans, and Public Policy.
  • Proven ability to translate detailed information into a clear, concise summary appropriate for executive consumption.
  • Practice Management training required; Certification preferred.
