The Manager, Quality Programs will manage the design, implementation and tracking of Tandigm’s portfolio of quality programs, which support the network’sprimary care practices in the delivery of quality healthcare.
The Manager, Quality Programs will stand up programs and define solutions to enhance Tandigm’s performance on key quality metrics measures and to improve outcomes for patients.The Manager of Quality Programs plans, manages and documents all aspects of the assigned projects. As a fast growing company, the Manager of Quality Programs plays a critical role at Tandigm by ensuring capabilities tied to overall key performance indicators are delivered on time, within scope and on budget.This position will partner with the Population Health Services Organization, Care Team, Tandigm Care Solutions, Accurate Coding and Education team, Informatics and other departments to achieve support all aspects of a project life-cycle.
•Responsible for promoting and engaging stakeholders in quality initiatives. Scope, plan, design, implement and manage programs, including establishing processes and data flows.
•Assist in the definition of project scope and objectives, involving all relevant stakeholders and ensuring technical feasibility.
•Create and maintain a detailed project plan to monitor and track progress.
•Facilitate the planning of recurring meetings, including scheduling, preparing meeting materials, coordinating and managing follow-ups.
•Understand, develop, track, monitor, and report on key quality performance metrics.
•Establish partnerships with preferred specialist groups and hospitals to improve screening rates and health outcomes
•Set up and manage quarterly patient mailing campaigns (statins, awareness months, etc.).
•Design, manage, and track impact of initiatives focused on patient satisfaction (analyzing survey results, contributing to content of training, tracking practice completion).
•Assist Tandigm Care Solutions team with implementing processes to drive performance on quality metrics.
•Scale up Tandigm’s advance care planning program.
•Create best practices and workflows to help practices improve internal processes.
•Working with informatics, analytics and IT departments, coordinate development of actionable reports for practices.
•Manage the development and delivery of trainings to internal and external parties, on topics such as best practices, work flows or provider satisfaction.
•Cooperate effectively with partners (payers, hospitals, vendors, community partners, etc.) to achieve shared goals.
•Keep abreast on industry standards and best practice methodologies related to healthcare quality, including the evolution of HEDIS measure specifications and CMS stars ratings.
•Appropriately engage the Compliance Department to ensure that programs maintain Tandigm’s compliance with federal, state, and/or local regulators and laws, and are within Tandigm’s contracted obligations to all stakeholders
•Consistently exhibit behavior and communication skills that demonstrate Tandigm’s commitment to superior customer service, including quality, care and concern with each and every internal and external customer
•Use, protect, and disclose Tandigm’s patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
