NURSE PRACTICTIONER/SKILLED NURSING FACILITIES
FIVE COUNTY PHILADELPHIA, PA REGION
(BUCKS, MONTGOMERY, PHILADELPHIA, DELAWARE, CHESTER)

The Post-Acute Nurse Practitioner is responsible for clinical evaluation and management of patients in a post-acute setting (skilled nursing facilities), as assigned by the Primary Care Physician, Medical Director, Hospitalist or Care Manager. The Nurse Practitioner provides diagnosis, treatment, consultation, and follow-up under the supervision of one or more physicians. The Nurse Practitioner provides age and specialty appropriate medical care at the level of training achieved and scope of practice. The Nurse Practitioner is accountable to utilize the appropriate level of care: working with Primary Care Physicians and specialists to facilitate immediate treatment and coordinate care to ensure quality patient care. The Nurse Practitioner is also responsible for managing the care of patients in the post-acute setting in the same way a primary care provider is responsible for managing outpatient care. The Nurse Practitioner is responsible to adhere to established parameters of the contracts and health plan benefits, and works collaboratively with nurse care managers, consulting and primary care physicians, and other facility based health care professionals to form a care team. The Nurse Practitioner helps optimize care through more effective practice and accelerates the patient’s progress by ensuring efficient yet compassionate care.

Essential Functions

  • Oversee and be accountable for overall quality of care, clinical outcomes, and resource utilization within the assigned post-acute facilities
  • Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues
  • Communicates with patient, patient’s family, and/or caregiver regarding treatment plan and initiates end-of-life discussions and hospice when deemed appropriate
  • Effectively identifies, evaluates and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care
  • Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications
  • Demonstrates, by action and attitude, the standard of high patient satisfaction, treating each patient with respect
  • Demonstrates, by action and attitude, the standard of high staff and physician satisfaction, treating staff and physicians with respect
  • Provide patient care in a manner consistent with facility bylaws, LOS standards, and evidence based practices
  • Attend and participate in all regularly scheduled team meetings, including daily care team meetings within the assigned facilities
  • Responsible for timely documentation, dictation of discharge summary and verbal communication back to patents’ PCP at time of discharge; completes all medical records within 24 hours
  • Ensure coding and documentation activities are met for completeness, accuracy, timeliness, and reimbursement level
  • Mentor and educate all new teammates on HCP program standards
  • Work with the Lead Clinician to assure proper provider staffing, including vacation coverage
  • Accountable to respond to urgent needs after hours when medically indicated
  • Night and weekend rounding will be shared by entire team, and schedule will be coordinated by Lead Clinician
  • Attends continuing medical education (CME) programs as needed
  • Participates in HCP activities such as health fairs, classes and community events, as required
  • Collaborates with other clinical and administrative staff in a positive, constructive manner
  • Meet all facility requirements and bylaws for Medical Staff
  • Meet all HCP provider credentialing and QI standards
  • Uses, protects, and discloses HCP patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Performs other duties and responsibilities as assigned or requested by the Lead Clinician
  • Consult with attending physician(s) as necessary

Education & Experience

  • Current RN and NP license in Pennsylvania; additional license in New Jersey preferred
  • Certificate of completion from an accredited Nurse Practitioner Program
  • Current DEA certificate
  • At least one-year experience practicing as a Nurse Practitioner;
  • Must have valid driver’s license and insurance and must have property damage insurance. Must be comfortable driving throughout the five county Philadelphia, PA region.

  • Carrying/Lifting: Occasional / 0-10 lbs
  • Standing: Occasional / Up to 3 hours per day
  • Sitting: Constant / Up to 8 hours per day
  • Walking: Occasional / Up to 1 hour per day
  • Repetitive Motion: Keyboard activity, telephone use, writing
  • Visual Acuity: Ability to view computer monitor and read newsprint with or without corrective lenses
