Part-time Population Health Services Analyst

Responsible for serving as the internal subject matter expert for Tandigm’s new value-based agreements with additional payers and will report into the Population Health Services Organization. This includes providing education, support, and oversight of the new payer performance programs while helping to implement best practices to align with Tandigm’s performance program. This position will support our primary care practices from both an internal and external perspective in order to help Tandigm’s goal of strengthening independent primary care through aligned practice workflows and enhanced incentives.

Essential Functions

  • Develop collaborative relationships with Population Health Services Organization (PHSO), Accurate Coding and Education (ACE), and Operations teams in support of driving performance in additional payer value based agreements.
  • Develop a proficiency in the value-based performance program and tools through training and education provided by the payer and other internal Tandigm resources.
  • Develop oversight of performance management through practice specific data analysis to determine improvement opportunities based on a core set of clinical, operational and quality measures.
  • Develop progress reports to share with PHSO provider facing team, management, and practices when necessary.
  • Support and assist PHSO team in education, application and tracking of performance status among participating primary care practices.
  • Explore and develop best practices to assist primary care practices in achieving success in the various value-based care performance programs in which they are participating.
  • Possess a proactive approach in resolving provider network issues with regard to payer agreements.
  • Maintain oversight on the use of payer enablement technologies and population healthcare management tools.
  • Provide education of enablement technologies to PHSO team as well as PCP practices participating in the payer value-based programs when necessary.
  • Act as a resource for PHSO team on an ongoing basis to answer questions and resolve issues as they arise pertaining to payer value-based agreements.
  • Assists PHSO team in developing model practice improvement plans to align contracted primary care physicians with company initiatives, goals and quality outcomes.
  • Support practice improvement on a core set of clinical, operational, and quality measures.
  • Provide education on new and evolving payment models.
  • Performs other duties as required.

Education & Experience

  • Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college and;
  • Minimum 3-5 years progressive experience in a health-care related organization is preferred, or previous experience in Population Health or value-based agreements is a plus; with
  • Previous project management experience is preferred.
