Part-time Pharmacy Consultant – House Calls

Our part-time house calls Pharmacy Consultant will join our growing team in the Philadelphia area focused on improving the healthcare of patients with the most intense and complex medical needs. This position will be responsible for performing all necessary functions to ensure optimal use of medication and provision of compassionate pharmaceutical care including advising patients and healthcare providers; collaborating with other healthcare professionals to plan, monitor, review and evaluate medication effectiveness; monitoring patients for side effects and drug interactions; and assisting with transitions of care. Must be comfortable providing assessments and counseling telephonically for a panel of patients in collaboration with house calls teammates and clinical staff.

Essential Functions

  • Independently perform medication audits and medication reconciliations to directly impact high-risk patients.
  • Provide concise, complete, applicable and timely responses to drug information requests from healthcare professionals and patients
  • Confer with our clinical team to correct problems with physician prescription or orders; recommend changes in therapy with the goal of enhancing patient care, increase patient safety and decrease costs; IV to PO recommendations, clinical practice guidelines, and antibiotic selections
  • Consult with patients regarding possible side effects of medications, proper administration of medications, potential drug/food or drug/drug interactions
  • Attend case review meetings with clinical staff and consult on patient medication plans
  • Independently provide counseling to patients in the outpatient setting telephonically and at discharge from hospital or SNF when necessary
  • Participate in the development of clinical practice guidelines and the implementation of clinical orders
  • Collaborate with other clinical and administrative staff in a positive, constructive manner
  • Use, protect, and disclose patients’ protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
  • Perform other duties and responsibilities as assigned or requested

Education

  • Doctor of Pharmacy degree from an accredited college of pharmacy
  • Pharmacist license in Pennsylvania
  • Board certification in Pharmacotherapy specialty preferred

Experience

  • Pharmacy residency or fellowship preferred
  • Strong communication skills and ability to provide direction telephonically
  • Excellent organizational and planning skills
  • Strong coordination skills and ability to work in a cross-functional team to manage complex patients
  • Experience working telephonically or with geriatric populations preferred
  • At least 4 years of experience as a clinical or consultant pharmacist or any combination of education and experience, which would provide an equivalent background.
  • Experience in patient counseling and medication therapy management preferred

  • Extensive knowledge of drugs, pharmacotherapeutics, diagnosis and disease states

  • Ability to establish and maintain working relationships with various members of the healthcare teams
  • Ability to communicate clearly in oral and written form with members of the healthcare team
  • Ability to work telephonically and/or in person with patients
