Population Health Liaison

Based in Conshohocken, PA. Requires daily driving between locations in Philadelphia, Montgomery, Chester, Delaware, & Bucks Counties.

In this role at Tandigm Health, you’ll have the opportunity to work with an extensive network of primary care practices, consulting with clinicians and their support staff on data-driven techniques for re-imagining the patient experience and improving healthcare outcomes in southeastern Pennsylvania (including Bucks, Delaware, Chester, Montgomery, and Philadelphia counties).  If you’ve previously worked in a healthcare-related role, enjoy working with a wide variety of people, and have strong communication skills as well as a penchant for process improvement, you could be the person we’re looking for to join the Tandigm Team!

  • The Population Health Liaison works hard to serve as a trusted population health expert for primary care practices as they make the switch from fee-for-service to value-based care.
  • Physician practices rely on this role to guide them through Tandigm’s programs that help deliver to patients “the right care at the right time in the right place.”
  • The Population Health Liaison helps physician practices implement new processes that can enhance the patient experience and overall patient engagement – crucial factors in improving healthcare outcomes.

Experience and Education

  • Minimum 3-5 years’ experience in a healthcare-related organization is required.
  • Primary Care Practice operations experience, direct knowledge of primary care practice operations, and/or experience with business process improvement is needed.
  • Associate’s degree or Bachelor’s degree – preferably related to healthcare and/or business – from an accredited institution of higher learning is required.

Licenses/Certifications

  • Valid U.S. State Driver’s License as well as proof of liability and property damage insurance on any vehicle used for business purposes are required.
  • Certified Medical Practice Executive (CMPE) credential, or equivalent, a plus.
  • Other healthcare-related professional certifications a plus.
  • Business-related professional certifications a plus.
