Population Health Specialist

The Population Health Specialist (PHSO Specialist) is a vital part of the Population Health Services Organization (PHSO) team, which focuses on maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network.  This role provides administrative, scheduling, and specialized technical support for the PHSO Associate Directors (AD’s) and their practices.  Although based in Conshohocken, this position will occasionally travel within the five Philadelphia counties to provide onboarding for new practices and deliver short training presentations to existing practices regarding Tandigm programs.

While this specialist role reports to the PHSO Director, it’s also a “dotted line” direct report to an assigned AD, with whom this role works closely on a daily basis.  The AD works primarily in the field and will rely on remote communication with the specialist, using phone, email, instant messaging, and texting, as needed and as appropriate.

The PHSO Specialist will assist and, as applicable, partner with an AD on developing and occasionally delivering presentations and reports to practices, updating them on their current performance metrics and advising on improvement processes.  This assistance and partnership will extend to providing appropriate support to the practices when the AD is out of the office or on paid time off.

The individual in this role is expected to possess a high degree of organizational skills and consistently exhibit behavior and communication skills that demonstrate Tandigm Health’s commitment to providing an excellent customer experience.  These skills are vital in order for the PHSO Specialist to provide technical support to practices for Tandigm’s online applications, assist with client dinners and network meetings, and to participate in special projects such as our Action Plan Library or advising on our multi-payer arrangements.

Essential Functions

  • Develops effective client relationships among our primary care physicians and their respective practices.
  • Possesses a proactive, problem-solving approach in handling provider network issues and, when necessary, helps to interface between Tandigm departments and the PCP network.
  • Conducts and assists with ongoing provider training in all Tandigm related programs, as needed.
  • Engages providers to support Tandigm’s quality improvement activities.
  • Provides tenacious, resourceful customer service for providers on an ongoing basis to answer questions and resolve issues as they arise.
  • Develops collaborative, internal relationships with various Tandigm teams to improve our overall customer service for the practices.
  • Assists the AD in brainstorming, researching, and investigating quality improvement and change methodologies, tools, best practices, and lessons learned in value-based care.
  • Provides practices with training on the use of enablement technologies and population healthcare management tools.
  • Contributes to, and maintains awareness of, the Action Library content, advising the AD accordingly when practice action plans are being developed.
  • Assists the AD with ongoing monitoring and feedback regarding practice performance with regard to Tandigm’s various performance metrics.
  • Prepares reports and presentations, under the AD’s guidance, as needed.
  • Manages the AD’s schedule for meeting and following up with practices.
  • Performs other duties as required.

Education

  • Bachelor’s degree from a four-year college and/or a professional certification requiring formal education beyond a two-year college.

Experience

  • Minimum 2-4 years progressive experience in a healthcare-related organization is preferred
  • Practice operations experience is preferred
  • Experience in a clinical environment a plus
  • Previous business development, sales, financial, and/or marketing experience a plus

Interpersonal Skills

  • Must be able to effectively influence stakeholders and peers without direct reporting relationships.
  • Must possess effective verbal and written communication and analytical skills, required to convey complex ideas and concepts.

Licenses/Certifications

  • Valid U.S. State Driver’s License, as well as proof of liability and property damage insurance on any vehicle used for business purposes, are required.
