, Holy Redeemer Health System, and Innovative Wellness Alliance (IWA) today announced a landmark agreement to improve the delivery of quality health care, reduce costs, and enhance patient satisfaction. Under the agreement, more than 75 primary care physicians from IWA, a physician-led, patient-centered organization, along with Holy Redeemer’s extensive network of inpatient, outpatient, home health, and long-term-care services, will collaborate with Tandigm Health and its growing network of primary care physicians. These physicians will benefit from Tandigm Health’s clinical expertise, analytic tools, and real-time data to help them better manage difficult chronic conditions, expedite preventive care, and deliver more cost-effective, high-quality care. As a result of this collaboration, Tandigm will have a network of more than 350 primary care physicians.
The deal marks Tandigm Health’s first cooperative arrangement with both a clinically integrated network of primary care physicians and a health system that provides a full continuum of healthcare services. The agreement will be effective March 1, 2015.
“Enthusiasm for our model among primary care physicians in our region has exceeded our expectations,” said Dr. Anthony Coletta, president and CEO of Tandigm Health. “This innovative agreement with Holy Redeemer and IWA truly accelerates our vision to improve quality and lower the costs of healthcare in one of the largest and most expensive healthcare markets in the United States. Together, we can ensure that patients are receiving the best care in the right settings and that the physicians and health system are rewarded for improving quality and efficiency.”
Tandigm Health, based in West Conshohocken, Pa. was formed in April 2014 by affiliates Independence Blue Cross and DaVita HealthCare Partners (NYSE: DVA). Tandigm Health enables the primary care physicians in its network to provide superior care to patients, emphasizing the quality of care, not the quantity of care.
IWA was formed in January 2014 as a clinically integrated network comprised of primary care physicians, nurse practitioners, and Holy Redeemer to improve quality and manage the cost effectiveness of care. Holy Redeemer is uniquely qualified to deliver excellent care in the most appropriate and cost-effective environment because it has facilities and services that cover the continuum of care from in-patient hospitalization to home care to long-term care.
“Forging this collaborative relationship among Tandigm, IWA, and Holy Redeemer will advance necessary change in our region to bring value to the healthcare consumer, employers and others who pay for healthcare services,” said Michael Laign, president and CEO of Holy Redeemer Health System.
“As primary care physicians, our goal is to keep our patients healthy and guide them to the most effective care they may need, from home care to hospital care. IWA’s agreement with Tandigm Health will give us greater access to the resources and processes we need to provide the highest standard of care,” said Dr. Carl Rosenbaum, president of IWA and vice president and medical director of Holy Redeemer home care and population management.
