Phildelphia, PA – August 11, 2016
Tandigm Health, a value-based healthcare company supporting primary care physicians, today announced collaborative partnerships with 65 new primary care physicians, all part of Quality Health Alliance-ACO, LLC (QHA), a physician-led quality care organization formed by Bucks County-based St. Mary Medical Center.
Through the new agreements, Tandigm Health welcomes primary care physicians from 24 independent practices and 15 employed practices associated with QHA into the Tandigm network. The participating QHA physician groups will now work hand-in-hand with Tandigm Health, allowing them to strategically unite as a leading force to provide innovative care solutions to improve care quality for the patient population that they serve. Both partners share similar missions and through this agreement are leading the movement to further advance the value-based care model. With this new relationship, these QHA physicians will receive access to the wide array of advanced tools, technology, resources and meaningful incentives that Tandigm provides and the partnership will further enable Tandigm Health to expand its transformational system of physician-led, coordinated care to a broader audience of patients and physicians across the Philadelphia region. Aldo Ciccotelli, MD, FAAFP, QHA’s Chief Medical Officer and a local physician, will also serve as part-time Medical Director of Tandigm, further solidifying the collaborative relationship between these two organizations.
“At Tandigm, we define healthcare quality as ensuring each individual has the right care, in the right place, at the right time – simply because it’s the right thing to do. That is why we are thrilled to work with the like-minded physicians at Quality Health Alliance, an organization that shares the same core values and mission as we do,” said Dr. Anthony Coletta, CEO of Tandigm Health. “Through our collaborative relationship with the Quality Health Alliance doctors, Tandigm is given the opportunity to provide the finest possible care to an even larger group of patients by expanding our proactive, coordinated care model to a new group of physicians.”
Tandigm was founded on the belief that primary care physicians are uniquely positioned to influence the quality and cost of care because they serve as the first-line of healthcare for most Americans and the company is dedicated to expanding its care model to serve more patients. The company sees great value in partnering with like-minded physicians and organizations in an effort to continuously advance its value-based model. These relationships, coupled with Tandigm’s strong first-year results, reinforce the value that comes from collaborating with hospitals, primary care physicians, patients and technologies.
“The partnership between Quality Health Alliance and Tandigm will help us close care gaps and provide integrated, person-centered care for the populations we serve,” said Benjamin Chack, MD, an independent ear, nose and throat specialist and President of Quality Health Alliance-ACO, LLC.
Tandigm Health has already begun working closely with participating QHA physician practices to familiarize them with Tandigm’s model and smoothly transition them into the Tandigm family. To learn more about Tandigm’s approach to value-based healthcare, visit www.tandigmhealth.com.
