Our care coordinators will work together as a team to facilitate the planning and organizing of all critical activities for our patients in the home setting. As a care coordinator you will facilitate referrals and assist our field based team with utilization and patient transitions back to their home.
Certified Professional Coders are responsible for serving as a key resource for medical coding matters. Performs reviews and audits and codes medical records to ensure the appropriate diagnostic codes and modifiers are used per Generally Accepted Medical Coding Guidelines, ICD-10 Guidelines and the CMS National Correct Coding Initiative. Participates in the implementation of the organization’s Coding Proficiency program. Interfaces and disseminates audit results to clinicians and management and provides guidance to practices on improving coding accuracy.
This position is responsible for ongoing development of the Tandigm Health Provider Network. This includes education, support, and oversight of the primary care physician network. This position will also manage the physician incentive program for assigned geographies and will work with the Tandigm primary care practices to help PCPs make greater connections to their patients, receive greater financial rewards, and positively affect the care delivered in the Philadelphia market.
Responsible for serving as the internal subject matter expert for Tandigm’s new value-based agreements with additional payers and will report into the Population Health Services Organization. This includes providing education, support, and oversight of the new payer performance programs while helping to implement best practices to align with Tandigm’s performance program.
