The Associate Director (AD) serves as a guide to physician practices in their transformation from fee-for-service to value-based care. As a population health subject matter expert, the AD consults with practices on process improvements to help physicians enhance the patient experience and contribute to a net positive impact on the healthcare delivered in the greater Philadelphia area.
The Field-Based Nurse is responsible for conducting clinical visits with Complex Care Management (CCM) patients who require home visits within the NE Philly/Montgomery County, PA territory. The Field-Based Nurse conducts nursing assessments to determine clinical status and patient needs, contributes to the patient care plan, and collaborates with the Nurse Care Manager to establish the frequency
of home visits.
Reporting to the Sr. Director, Quality Programs & Performance, the Healthcare Quality Business / Data Analyst will be responsible for interfacing between the Tandigm’s Quality and Information Systems and Technology (IS&T) teams to manage the technical aspects of quality measurement in order to ensure the accuracy of the data and reports, used both internally and externally (by practices and payors). This position will develop, document and manage business requirements and will partner with Tandigm’s IS&T team, vendors and payors to create and oversee quality assurance processes related to quality reporting and incentives payment.
The Internal Auditor, based out of our West Conshohocken, PA office (remote until further notice), will independently perform audits and other projects as assigned by the Director of Compliance. Such work includes, but is not limited to, completing audits for Tandigm Health and its affiliates to test that controls are in place and functioning as intended and business practices are effective, efficient, and compliant with Tandigm policies and regulatory requirements.
The primary responsibility of the Nurse Care Manager is to telephonically design and oversee the delivery of care plans for each patient in their panel. Serving as the primary point of contact for their patients, the Nurse Care Manager partners with all members of the Complex Care Management (CCM) team by gathering clinical input, facilitating team discussions, and coordinating all elements of their patients’ care plans. The Nurse Care Manager is also the telephonic central point-of-contact for their assigned PCP’s, maintaining positive relationships between the PCP’s and Tandigm Health. Position is remote after training.
Under the direction of the VP, Clinical Network Transformation, the Senior Director, Network Transformation will help to drive the transformation initiatives and evolution of Tandigm’s High Value Network. This will include support of network transformation strategies, practice recruitment into the Specialty Alliance, and operational program management of the Specialty Alliance and its efforts toward clinical integration and practice transformation to achieve the goals of Tandigm.
