This position is accountable for maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network. The Associate Director (AD) will be assigned a geographic region within the five county Philadelphia area and hold accountability for the performance management, customer service, and training for all of the Tandigm Health private practices in that region. The AD will need to accommodate client availability for regular meetings with a flexible work schedule Monday through Friday and can expect to spend an average of 20-30 hours “on the road” each week visiting practices. Every candidate must have a valid U.S. State Driver’s License and personal vehicle for their own use.
The Director, Clinical Network Transformation, will help drive transformation initiatives and the evolution of Tandigm’s Specialty provider engagement model, supporting network transformation strategies and practice recruitment into the Specialty Alliance. Tandigm Health, a transformational leader in population health management, is looking for a Primary Care Network Solutions Specialist to support our Associate Directors, Leads, and Director who are responsible for maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network. You will troubleshoot practice questions, assist with developing and preparing presentations and reports, engage with practices via telephone/video conferencing and email and may be asked to attend practice meetings. You will provide support to the practices and be responsible for the coordination and facilitation of internal segment meetings.
Our Nurse Practitioner is a member of our Complex Care Team, committed to providing home-based care to adult patients with advanced illness and limited accessibility to healthcare. In this role, you will work directly with the Nurse Care Manager to provide insights drawn from patient visits to inform appropriate changes to the care plan. You will also partner with the patient's primary care medical providers addressing preventative health care and quality gaps in care, educate patient and family members, identify additional diagnoses, and provide post-discharge transitions of care. You will bring your 2+ years’ of clinical experience managing patients with complex and chronic conditions to this collaborative role as you work with the Tandigm Team and the patient’s other healthcare providers to deliver patient-centered care within the Tandigm value-based model.
