This position is accountable for maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network. The Associate Director (AD) will be assigned a geographic region within the five county Philadelphia area and hold accountability for the performance management, customer service, and training for all of the Tandigm Health private practices in that region. The AD will need to accommodate client availability for regular meetings with a flexible work schedule Monday through Friday and can expect to spend an average of 20-30 hours “on the road” each week visiting practices. Every candidate must have a valid U.S. State Driver’s License and personal vehicle for their own use.
You will provide Tandigm Health’s patient-facing staff, patients, and PCP offices with effective communication and expert logistical support related to Tandigm’s services.
The Clinical Documentation Nurse – RN (Accurate Coding and Education Chart Reviewer) will perform thorough chart reviews to inform providers of potential inferred, embedded, and missing diagnoses not previously identified. In this role, you will be a team player and excellent communicator, who works closely with practices to review medical records to ensure continued high standards in documentation and coding.
As a Nurse Care Manager with our Complex Care Management Team, you will use your strong organizational and planning skills to design and oversee the delivery of care plans for each patient on your panel. In this telephonic role, you will serve as the primary point of contact for your patients, creating meaningful relationships to maximize the likelihood of patient adherence to the established care plan.
Our Senior Director will engage with all areas of Tandigm’s business to ensure the successful planning, execution, and delivery of initiatives aligned with the organization’s objectives. In this role, you will manage the development and end-to-end execution of a portfolio of strategic initiatives/programs in support of Tandigm’s annual operating plan.
In this role, you will use your strong analytical skills to identify trends and variances, develop models for planning/forecasting, and draw on your top-notch presentation skills to communicate this information to the team. You are a seasoned professional with a background in financial planning that will guide you as you perform analysis of the Tandigm Incentive Program performance results and identify performance program KPIs.
