This position is accountable for maximizing the clinical, operating, and financial performance of Tandigm’s valued primary care physician network. The Associate Director (AD) will be assigned a geographic region within the five county Philadelphia area and hold accountability for the performance management, customer service, and training for all of the Tandigm Health private practices in that region. The AD will need to accommodate client availability for regular meetings with a flexible work schedule Monday through Friday and can expect to spend an average of 20-30 hours “on the road” each week visiting practices. Every candidate must have a valid U.S. State Driver’s License and personal vehicle for their own use.
Our Behavioral Health Consultant functions as part of an interdisciplinary team to improve the health of patients with the most intense and complex medical needs. In this role, you will provide a combination of in-home and telephonic assessment, diagnosis, and therapeutic interventions to patients with identified or suspected behavioral health conditions. You will collaborate with Tandigm Medical Directors, Nurse Practitioners, Nurses, Social Workers, patients and their families, and other healthcare professionals to address the patient’s behavioral health needs. You will bring to this role your 5+ years of professional clinical experience providing behavioral health services to adults and older adults; you are comfortable working in non-traditional behavioral health settings.
Our Communications Manager plays a critical role in developing, executing, and optimizing communications, marketing, and social media strategies at Tandigm Health. In this role, you will draw on your 5+ years’ experience in healthcare communications and marketing to drive brand awareness and engagement. Your expertise in managing communications will be evident as you collaborate with multiple departments and translate complex clinical projects to clear and concise messaging targeting Tandigm practices, staff, media, and the public. You will write communications plans and content in a range of formats, track outcomes, and recommend strategies to increase engagement. You have the creativity to explore new solutions and the initiative to get projects across the finish line while managing multiple tasks.
Our Field-Based Nurse is a seasoned professional with over 5 years of relevant clinical experience in a hospital, clinical, or home care setting who conducts in-home clinical visits with our Complex Care Management (CCM) patients. In this role, you will conduct nursing assessments of patients to determine their overall health status and provide clinical recommendations to the Nurse Care Manager to inform the development of care plans. You will also partner with other CCM members (e.g., Nurse Practitioners, Social Worker, BH Specialist, Pharmacist) to ensure all factors impacting the patient’s clinical well-being are being addressed through the care plan.

As Lead Scrum Master, you will be responsible for facilitating the coordination and collaboration between multiple Scrum teams, ensuring efficient and effective delivery of complex projects. You will play a pivotal role in leading the agile transformation, ensuring successful project execution and alignment with business objectives. You have 8+ years of experience as a Scrum Master and are an expert at identifying and addressing project risks, working closely with stakeholders to develop mitigation plans as needed to ensure timely resolution. You are skilled at tracking and reporting on the progress of your teams, adhering to sprint schedules, delivery commitments, and quality standards. You are a coach and mentor on Agile principles, providing guidance and encouraging self-organization, collaboration, and accountability within and across Scrum teams.
The Part-time Utilization Management Medical Director is a licensed physician responsible to provide Medical Director reviews of requests made to the Utilization Management department to maximize quality of patient care and ensure appropriate utilization of resources within the parameters of health plan benefits and established contracts.
Our Post-Acute Clinical Care Liaison is responsible for the development and expansion of our post-acute management program, establishing relationships and facilitating processes with local hospitals. In this role, you will educate hospital administration, physicians, and discharge planners on the high-quality facilities available for post-acute care. Utilizing Tandigm’s patient lists and EMR alerts, you will proactively identify patients who might benefit from Tandigm programs.
In this role, you will be responsible for ensuring the continuity of patient care in the post-acute setting (skilled nursing facilities) utilizing the appropriate resources within the parameters of established contracts and patients’ health plan benefits. You will facilitate the continuum of patients’ care utilizing your advanced nursing knowledge, experience, and skills to ensure appropriate utilization of resources and patient quality outcomes. As part of a care team, you will collaborate with nurse practitioners, care managers, consulting and primary care physicians, and other facility-based health care professionals to help optimize care through more effective practice and accelerate the patient’s progress by ensuring efficient yet compassionate care.
Our Senior Application Support Analyst will be a dedicated subject matter expert for Tandigm’s data applications. In this role, you will use your top-notch trouble shooting and problem-solving skills to tackle bug fixes, minor software enhancements, and configuration management. You will also work closely with third party resources to provide vendor management and support Tandigm’s Population Platform customer base and custom integrations.
Our Test Architect is a member of the Technology Architecture team that designs and implements the overall test strategy and architecture at Tandigm Health. In this role, you will collaborate with cross-functional teams to identify test requirements, develop test plans, define test automation frameworks, and ensure the overall quality and efficiency of the testing process.
