The Clinical Pharmacist will be responsible for performing all necessary functions to ensure optimal use of medication and provision of compassionate pharmaceutical care including: advising patients and healthcare providers; collaborating with other healthcare professionals to plan, monitor, review and evaluate medication effectiveness; monitoring patients for side effects and drug interactions; and assisting with transitions of care.
The Community Based Nurse Navigator (CBNN) is accountable for the high-risk patient population of Tandigm PCPs. As such, s/he takes responsibility for ensuring the continuity and coordination of patient care delivery by assessing patient needs; developing (in collaboration with the patient, patient family, and PCP) a Plan of Care (POC) that addresses those needs; identifying and leveraging appropriate resources (within the parameters of established networks, programs, and policies) to implement the POC; and regularly evaluating patient progress against the POC.
The Director, Population Health Services Organization (PHSO) is responsible for overseeing, managing and development of the team of Associate Directors, Liaisons, and Specialists that comprise the PHSO. This responsibility includes oversight & development of appropriate strategies and action plans for Tandigm PCPs and their staff, focusing on driving their clinical, operating, and financial performance consistent with Tandigm’s goals and objectives.
The Project Manager is responsible for managing enterprise-wide projects. This role drives projects that deliver capabilities in support of Tandigm’s Clinical Networks, Clinical, Informatics, Finance, and Accurate Coding and Education (ACE) business units. As a fast growing company, the Project Manager plays a critical role at Tandigm by ensuring capabilities tied to overall key performance indicators are delivered on time, within scope and on budget.
The Nurse Chart Reviewer is responsible for performing thorough chart reviews in order to inform providers of potential inferred, embedded, and missing diagnoses not previously identified and to aid in the education and discovery of opportunities for improved coding and documentation.
