The Community Based Nurse Navigator (CBNN) is accountable for the high-risk patient population of Tandigm PCPs. As such, s/he takes responsibility for ensuring the continuity and coordination of patient care delivery by assessing patient needs; developing (in collaboration with the patient, patient family, and PCP) a Plan of Care (POC) that addresses those needs; identifying and leveraging appropriate resources (within the parameters of established networks, programs, and policies) to implement the POC; and regularly evaluating patient progress against the POC.
The Executive Assistant provides critical support to the President & Chief Executive Officer, Chairman and Senior Management Team and provides administrative functions essential to the mission of the organization and the day-to-day-operation of the Executive Office.
Responsible for serving as the internal subject matter expert for Tandigm’s new value-based agreements with additional payers and will report into the Population Health Services Organization. This includes providing education, support, and oversight of the new payer performance programs while helping to implement best practices to align with Tandigm’s performance program.
As part of Tandigm Health’s Palliative Care Program, the Spiritual Counselor (within the Five County Philadelphia, PA territory) provides and oversees the spiritual needs of Tandigm’s palliative care patients and families.
This part-time position will be responsible for performing all necessary functions to ensure optimal use of medication and provision of compassionate pharmaceutical care including advising patients and healthcare providers; collaborating with other healthcare professionals to plan, monitor, review and evaluate medication effectiveness; monitoring patients for side effects and drug interactions; and assisting with transitions of care.
Reporting to the Director, Practice Growth and Strategy, the Practice Growth Account Representative is responsible for building community partners for referrals to Tandigm Health's practices and assist in meeting the membership goals of the department.
The Receptionist/ Administrative Assistant will be responsible to greet visitors, answer the phone, retrieve departmental voicemail, schedule meetings, and perform all departmental administrative duties including filing, copying, opening & distributing mail.
