The Accurate Coding and Education (ACE) Chart Reviewer is responsible for performing thorough chart reviews for our participating Primary Care Physician Practices to inform providers of potential inferred, embedded and/or missing diagnoses not previously identified and to aid in the education and discovery of opportunities for improved documentation and coding.
Certified Professional Coders are responsible for serving as a key resource for medical coding matters. Performs reviews and audits and codes medical records to ensure the appropriate diagnostic codes and modifiers are used per Generally Accepted Medical Coding Guidelines, ICD-10 Guidelines and the CMS National Correct Coding Initiative. Participates in the implementation of the organization’s Coding Proficiency program. Interfaces and disseminates audit results to clinicians and management and provides guidance to practices on improving coding accuracy.
The Community-Based Nurse Navigator (CBNN) uses advanced nursing knowledge, evidence-based best practice, critical thinking skills, and excellent oral and written communication skills to ensure appropriate utilization of resources and patient quality outcomes. The CBNN candidate brings a passion for the work, a belief in the role, and a commitment to the patients and physicians served.
The Health Informatics Analysts work with stakeholders from across the company to help them answer critical business questions. The Health Informatics Analysts for Tandigm Health work on a variety of projects. They analyze medical claims data sets to discover patterns and uncover new opportunities. The Health Informatics Analysts design new reporting packages; create specification documents and help build and deploy them. They are responsible for insuring data quality and appropriate data use to comply with HIPAA (Health Insurance Portability and Accountability Act) regulations.
The Quality and Performance Improvement Specialist will support the Director of Quality in managing the implementation of Tandigm’s portfolio of quality programs, which assist the network’s primary care practices in the delivery of quality healthcare.
The SQL Developer will be responsible for ETL integration and application development in a Microsoft SQL Server platform. This will include reports, vendor exports, data warehousing tasks and other data related tasks. This position requires excellent communication, technical and project management skills.
