Tandigm Value Partners ACO Ranks in Top 25% Nationwide

West Conshohocken, PA –  Tandigm’s first Accountable Care Organization (ACO), Tandigm Value Partners (TVP) generated significant savings in 2022, its first year of operations, ranking it in the top 25% of overall ACO program performance nationwide.

Data shared by CMS shows that while caring for 11,000 patients, Tandigm Value Partners generated 6.3% in savings over the plan year benchmark. This equals $8.5 million in savings for CMS, of which $6.2 million was shared with the ACO.

TVP achieved supporting performance strategies and metrics including evaluating more than 50% of patients for practice care management services, providing annual wellness visits for 81% of patients, practice participation in prospective chart reviews, and completion of accurate coding education. To maintain high performance standards, TVP practices provide exceptional quality care through patient visits, addressing quality gaps, and effectively managing their high risk patients.

CMS’ Medicare Shared Savings Program allows ACOs formed by groups of doctors, hospitals, and other health care providers to collaborate and provide coordinated, high-quality care to people with Medicare. ACO’s focus on delivering the right care at the right time while avoiding unnecessary services and medical errors. When an ACO succeeds in both delivering high-quality care and spending health care dollars more wisely, the ACO may be eligible to share in the savings it achieves for the Medicare program. This initiative also drives lower health care costs for people with Medicare, who see lower out-of-pocket spending on avoidable health care (such as emergency department visits) because the ACO has better coordinated their care.
