In this role, you will play a crucial role in maximizing the quality-of-care performance and collaboration among healthcare agencies and facilities in the Tandigm population service area. Your primary focus will be on high-impact Hospitals, Skilled Nursing facilities, and Home Healthcare agencies.
As the High-Value Network Associate Director, you will be responsible for ensuring performance outcomes, managing relationships, and providing ongoing support to organizations in the continuum of care. Your goal will be to foster the right care at the right place with optimal transitions for the Tandigm population.
You will guide facility and agency leadership in their transition from fee-for-service to value-based care as a population health subject matter expert. This will involve consulting on process changes that positively disrupt the status quo, leading to improved patient experience, reduced unnecessary medical spend, and better outcomes, such as lower readmission rates. Additionally, you will organize and facilitate Joint Operating Committees and work with analytics to bring actionable data to stakeholders, supporting process improvement initiatives linked to improved patient healthcare outcomes.
