Tandigm Launches Penn Medicine Healthcare Partners Accountable Care Organization

West Conshohocken, PA – January 2, 2024 – As part of the Medicare Shared Savings Program, Tandigm has launched its largest accountable care organization (ACO), Penn Medicine Healthcare Partners (PMHP). This new ACO includes 32,000 patients in primary care practices at more than 290 locations across the Penn Medicine network.

The PMHP ACO represents an expanded geographical footprint, with practices in the five-county Philadelphia region and in the Princeton, New Jersey region. This ACO is Tandigm’s third, having launched Tandigm Value Partners ACO with 11,000 patients in January 2022 and Doylestown Value Partners with an additional 12,000 patients in January 2023. The addition of PMHP brings Tandigm’s ACO network to more than 57,000 members.

A key initiative of the Centers for Medicare and Medicaid Services, (CMS) the Medicare Shared Savings Program allows health systems and doctors’ groups, acting through an Accountable Care Organization (ACO), to earn annual bonuses if their Medicare patient care cost is less than government projections.
