Philadelphia, PA – October 6, 2016 – Tandigm Health has become a member of HealthShare Exchange of Southeastern Pennsylvania (HSX), expanding the value-based healthcare company’s access to data that can improve patient care and outcomes and lower healthcare costs. The data provided by HSX will enable physicians in the Tandigm network to securely receive near real-time notifications about their patients from member hospitals, including alerts about emergency room visits, along with hospital admissions and discharge information – further enhancing the ability of these clinicians to provide proactive, coordinated care.
Tandigm focuses on engaging, enabling and empowering its network of primary care physicians, providing meaningful incentives for quality care rather than the volume of services, so that patients receive the best care at the right points and locations of delivery. Tandigm provides tools, technology and support that enable participating doctors to better understand the needs of their entire patient population and deliver the finest possible care. The nonprofit HSX is a health information organization, supported by some 40 hospitals and three health plans, as well as ambulatory practices and specialty facilities, serving Philadelphia and its surrounding counties. The information received through HSX will enhance the ability of doctors to reduce unnecessary emergency-department admissions and other hospitalizations. Tandigm is now using admission-discharge-transfer (ADT) data from HSX member hospitals and will begin to distribute near real-time notifications in the form of actionable alerts to its physician network in the coming months.
