Tandigm Health, a value-based healthcare company supporting primary care physicians, today announced collaborative partnerships with 65 new primary care physicians, all part of Quality Health Alliance-ACO, LLC (QHA), a physician-led quality care organization formed by Bucks County-based St. Mary Medical Center.
Through the new agreements, Tandigm Health welcomes primary care physicians from 24 independent practices and 15 employed practices associated with QHA into the Tandigm network. The participating QHA physician groups will now work hand-in-hand with Tandigm Health, allowing them to strategically unite as a leading force to provide innovative care solutions to improve care quality for the patient population that they serve. Both partners share similar missions and through this agreement are leading the movement to further advance the value-based care model. With this new relationship, these QHA physicians will receive access to the wide array of advanced tools, technology, resources and meaningful incentives that Tandigm provides and the partnership will further enable Tandigm Health to expand its transformational system of physician-led, coordinated care to a broader audience of patients and physicians across the Philadelphia region. Aldo Ciccotelli, MD, FAAFP, QHA’s Chief Medical Officer and a local physician, will also serve as part-time Medical Director of Tandigm, further solidifying the collaborative relationship between these two organizations.
