
Tandigm Health Accountable Care Organizations (ACOs) Saved Medicare More Than $32 million in 2024
West Conshohocken, PA — September 3, 2025 — Tandigm Health, a leading population health management company, announced today that in 2024 its Tandigm Value Partners accountable care organization (ACO) generated total savings for the Centers for Medicare and Medicaid Services (CMS) of $22 million,[1] with a shared savings rate of 13.4%. This represents a significant improvement from the 2023 savings rate of 8.4%. In its third year of operation, the ACO provided care for 11,500 area patients and was in the top 5% of ACO performance nationwide.
The Medicare Shared Savings Program makes primary care practices and delivery systems, organized as an ACO, accountable for the cost and quality of care and rewards them for performance in these areas.

In addition to Tandigm Value Partners, Tandigm Health’s two other ACOs also earned shared savings:
- Doylestown Value Partners provided total savings to CMS of $3.3 million, with a savings rate of 2.0% and earnings of $2.4 million for ACO participants. 2024 was the ACO’s second year of operation, serving 12,600 beneficiaries.
- Penn Medicine Healthcare Partners saved CMS $7.1 million with a 1.6% savings rate and earnings for ACO participants of $5.2 million, a notable accomplishment for an ACO in its first year of operation. This ACO served 32,800 patients.
