West Conshohocken, PA — October 15, 2026 – Tandigm Value Partners, an Accountable Care Organization managed by Tandigm Health, has received the 2026 Excellence Award from the National Association of Accountable Care Organizations (NAACOS). The award recognizes Tandigm Value Partners and participant TriValley Primary Care for their outstanding commitment to and accomplishments in accountable care.

Tandigm Value Partners generated total savings for the Centers for Medicare and Medicaid Services (CMS) of $22 million in 2024, with a shared savings rate of 13.4% ranking it in the top 5% of ACO performance nationwide. The ACO is now in its fifth year of operations and provides care for 11,500 patients in southeastern Pennsylvania. TVP and its participating practices closely manage patients to focus on prevention and quality metrics, with the goal of reducing unnecessary hospitalizations and emergency room visits. TVP’s success demonstrates that rewarding primary care providers for managing both the cost and quality of care can deliver meaningful results.
TriValley Primary Care, the largest practice participant in Tandigm Value Partners, cares for more than 9,000 Medicare beneficiaries. This multi-practice group has distinguished itself through a scalable, team-based approach to value-based care that combines physician leadership, care management, and population health expertise across practice sites. Strategic partnerships with continuing care retirement communities and the adoption of AI-enabled technology have strengthened care coordination, reduced administrative burden, and improved outcomes for older adults.
