High & Rising Risk is an enhanced Arcadia-supported population health solution that helps primary care practices identify, review, and intervene with patients most likely to benefit from early care management and clinical support.
The solution was recognized as an innovative use of the Arcadia platform, serving not only as a reporting tool but also as a practice-facing intervention engine that helps translate insights to action.
Tandigm launched this enhanced version of the High & Rising Risk tool in early 2026. The updated solution offered a redesigned user experience, streamlined workflows, a dynamic patient registry, and a proprietary risk stratification algorithm customized to Tandigm’s patient population and intervention programs. The algorithm combines clinical, utilization, and social risk factors to identify patients who may be at risk for future avoidable utilization and whose outcomes can be improved through targeted intervention.
