Navigating the Next Era of Quality: Policy Shifts, Practice Readiness, and Patient Engagement
Stay ahead of upcoming CMS and NCQA changes including updated clinical and digital measurements. Using blood pressure as an example, we will highlight where new metrics create the greatest opportunities to improve performance and outcomes. Practice leaders will also learn about strategies that can improve patient engagement using clear communications and health literacy tools.
Success in Risk Accuracy
This session will explore best practices to operationalizing risk accuracy and also discuss relevant regulatory updates. You will gain practical insight into how consistent documentation and workflows support a more complete patient story and strengthen both clinical and financial sustainability.
High and Rising Risk Population Management
Improve care outcomes by proactively identifying and engaging high‑risk and rising‑risk patients before health needs escalate. Practice leaders will learn how targeted interventions, care management strategies, and consistent review of high‑risk populations can reduce avoidable utilization, improve the patient experience, and support long‑term population health success.
Access for Better Care and Reducing Avoidable Utilization
Access is a critical driver of patient outcomes. Learn about practical strategies for access, proactive scheduling of Comprehensive Visits, and ensuring high‑risk patients with chronic conditions are seen more frequently. Practice leaders will learn how intentional access design supports timely care, strengthens continuity, and reduces avoidable emergency department visits and hospitalizations.
